We see our Psychiatric Support system as a wheel. The center of the wheel is the individual and our top priority is creating

stability within that center.

 

We maintain and improve well-being through many sources, which become the spokes of the wheel.

Within the first 72 hours of arrival in the Psychiatric Assisted Living Facility, our Care Managers (CM) provide a 90 minute comprehensive ‘Meet & Greet’ with each Client (and their support system/family members as requested). 

With the information gleaned in this meeting, individualized care plans are designed to manage medical, social and mental health needs.  Attainable goals are identified and molded into measurable care plans.

Residents meet with their CM for 30-60 minutes weekly to monitor progress on goals and required action items.


As we all know, there is no ‘one straight path’ to well-being. We are ever-evolving in our

mind, body and soul.

 

So together, we make adjustments as needed. We monitor every aspect of our Client’s well-being and draw from an integrative, effective combination of psychiatric support offerings:

  • Proper medical care in place for total well-being - PCP, psychiatric services, group therapies.

  • Medical appointment escorts to provide transport, take detailed notes and advocate for quality care.

  • Medication management and/or guidance. 

  • Overall Wellness. Proper nutrition, physical activity, community outings, and relaxation/anti-anxiety tools such as yoga, massage & animal/horticulture therapies. 

  • Spiritual practices. Daily meditation, self-motivating mantras, acupuncture. 

  • Intellectual stimulation. Continuing education programs at a local university and brain-engaging games and activities in the home.

  • Financial support. Connect with resources/programs available to each individual. 

  • Support system development for on-going stability.

Beyond goal development and maintenance, our CM’s assist each resident by matching them with appropriate services offered within SD County - whether it’s medical care, insurance needs, out-patient treatment or work and/or study programs, we guide and connect to proper tools and resources.
